If you are the victim of a crime that occurred on campus, you can submit your initial crime report here. This form should only be used for non-emergency “cold” reports with no evidence. If you feel there is evidence, such as cut bike locks, fingerprints, footprints, tool marks, etc., please call us at 491-6425 and an officer will respond.
Please use this form for: theft, bike theft, theft from auto, vandalism and suspicious circumstances. This form can also be used for "lost property" but only valuable items such as wallets, purses, jewelry or passports.
Please complete this form as accurately as possible. This will assist us in reviewing your report for a potential investigation. If you have partial information, such as an incomplete license plate number or a vague personal description, please provide as much information you have.
If you need to report more than one crime or multiple instances, complete a separate form for each report. Since this report is sent via e-mail, do not include any financial, social security or sensitive information. An officer will obtain that information if required.
IMPORTANT: Reporting a crime that is false or malicious is punishable by law. All violators will be prosecuted to the fullest extent for filling a false crime report to law enforcement authorities.
